Job Description Insight Global is seeking a Remote Pharmacist to join a strong team of clinicians, analysts, and clinical support staff at a healthcare startup. The purpose of this position is to provide an expert level medication therapy consultation and med reconciliation to the patients and providers and to help supervise the Clinical Care support team. This candidate will have a direct impact on patient care and the delivery of our core clinical services, stay continuously up to date on developments in the fast-moving drug field, and be aware of emerging patterns, treatment guidelines, and best practices. The Clinical Pharmacist receives assignments and functions under the supervision of the Head of Operations to carry out and supervise direct patient care responsibilities and perform a range of functions that include the maintenance and review of patient medication profiles; counseling patients on appropriate drug use; promoting adherence; developing clinical recommendations; reviewing completed medication reviews by other Virtual Pharmacist users; as well as interacting with other health professionals to help improve patient outcomes. Skills and Requirements -Actively Licensed as a Pharmacist in Virginia, Georgia, or Tennessee (Pharmacist will be limited to assignment within appropriate scope of licensure depending on state they are licensed in) -Experience handling 45+ outbound calls per day -2+ years of Med Reconciliation experience -Ability to review and assess the appropriateness of prescriptions with respect to age specific, disease state specific considerations -Ability to engage with the patients, promote adherence and treat them with empathy and compassion -Knowledge of chronic disease states, such as diabetes, heart failure, COPD, asthma, cardiovascular disease, hypertension, dyslipidemia, anxiety, depression, bipolar disorders, and thought disorders -Experience working with patients of different cultural, religious, and socio-economic backgrounds -Has knowledge of influence of patient's age, sex, concomitant disease states, concurrent drug therapy, foods and diagnostic procedures on drug activity and disposition -Has knowledge of local, state and federal regulatory requirements related to areas of functional responsibility -Willingness and ability to obtain registered pharmacist license in other states, as needed -Excellent written and oral communication skills -Strong computer skills, including proficiency in the Microsoft 365 and Google Workspace null We are a company committed to creating diverse and inclusive environments where people can bring their full, authentic selves to work every day.
