Description of the Role:
We are seeking a compassionate and dedicated Home Health Registered Nurse Case Manager to join our team on a part-time basis. In this role, you will be responsible for telephonic care management and support to patients in Allentown, PA and the surrounding counties.
Responsibilities:
- Reviewing patient assessments and developing individualized care plans with the care team
- Ensuring all skilled nursing care, including wound care, medication management, and IV therapy is provided as ordered
- Coordinating and communicating with healthcare team members to ensure comprehensive and effective care is provided
- Monitoring patient progress and adjusting care plans as needed
- Educating patients and their families on proper care techniques and self-management
- Documenting Transfer and Discharge Oasis as needed
Requirements:
- Valid Registered Nurse (RN) license in the state of Pennsylvania
- Minimum of 1 year of experience in home health or similar setting
- Minimum of 1 year of experience in a case management role
- Excellent clinical and critical thinking skills
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation
