Job Description
Job Responsibilities:
- Perform telephonic comprehensive medication reviews, which include but is not limited to ensuring a complete medication reconciliation is completed and performing clinical assessments on applicable conditions for each targeted member.
- Identify, document, and facilitate resolution of identified medication related problems.
- Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy.
- Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems.
- Build and maintain positive relationships with internal and external customers including physicians and nurses
Skills:
- Applicants must reside and have an active pharmacist license in either HI, TX, IL, FL, or PA
- Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
- Basic computer skills and proficiency with Microsoft Office products.
- Strong organizational skills, interpersonal skills, and detail orientated.
- Experience maintaining focus and meeting goals in a work from home environment.
- Previous call center experience
Education/Experience:
- Bachelor’s Degree in Pharmacy or PharmD from an accredited school of Pharmacy
