We’re seeking a tech-savvy RN Patient Care Coordinator licensed in New York to provide telephonic care to chronically ill patients. You'll conduct telephonic outreach, develop personalized care plans, and support patient health through virtual coaching. Collaborate with healthcare systems to ensure care continuity and improve patient outcomes. 

Essential Functions:  

• Manage and support patients’ healthcare needs via phone calls. 
• Utilize EHRs and other online platforms for documentation. 
• Develop and implement care plans for physical, mental, and preventative needs. 
• Coach patients on health goals and connect them with resources. 
• Handle administrative tasks, including medical records and insurance reviews. 

Work Environment:  

• Remote (work from home) position requiring a high-speed internet connection and a quiet, HIPAA-compliant workspace. 
• Full-time, 40 hours per week, Monday-Friday.
