Job Description

Position Overview:
We are seeking an experienced and compassionate Licensed Clinical Social Worker (LCSW) to support individuals and families through clinical assessments, counseling, and case management. The LCSW will serve as an integral part of the multidisciplinary care team to address psychosocial needs and improve client outcomes.



Key Responsibilities:

  • Perform biopsychosocial assessments and develop treatment plans
  • Provide psychotherapy services to individuals, families, and groups
  • Offer case management and connect clients to community resources
  • Support patients coping with chronic illness, trauma, grief, and crisis situations
  • Maintain thorough and accurate clinical documentation
  • Advocate for client needs and ensure culturally competent care
  • Coordinate with medical staff, school personnel, legal systems, and social service agencies
  • Conduct discharge planning and follow-up care arrangements



Qualifications:

  • Master's degree in Social Work from an accredited program
  • Current LCSW license in current State of residence
  • 2+ years of post-licensure clinical experience preferred
  • Experience with diverse populations and settings
  • Strong ethical judgment and empathy
  • Knowledge of local resources and systems of care



Work Environment:

  • Clinical office or virtual setting
  • Exposure to emotionally intense client situations
  • Collaboration with interdisciplinary teams
  • Variable schedule including potential home visits
  • Moderate travel may be required for community-based work
